Looking for a change of scenery and a chance to help others. Join a travel Physical Therapist role supporting home health patients in Lake Hamilton, AR. You will evaluate functional status, develop and implement physical therapy plans of care, document per CMS and agency policy, instruct patients and caregivers, and coordinate with the interdisciplinary team to meet patient goals. This role includes travel within the agency service area and nearby partner agencies and may involve weekend rotation and observed holiday availability. You will provide supervision of PTAs and, when permitted by state regulation, home health aides. Maintain state licensure and complete required documentation and progress notes according to agency policy.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Arkansas physical therapy license required to start the assignment.
• Home Health and OASIS experience required.
• CPR certification required.
• BLS certification from AHA or ARC may be required.
• Ability to complete evaluations and develop PT plans of care within state-specific guidelines and to report outcomes within 24 hours of evaluation.
• Ability to travel within the agency service area, including Malvern and Hot Springs coverage areas.
• Schedule flexibility to participate in weekend rotation and potential holiday availability as needed.

Preferred Qualifications

• Experience with HCHB (Homecare Homebase) electronic documentation.
• Prior supervision experience of PTAs and home health aides in accordance with state regulation.

Assignment Details

• Shift hours: 2000-0400.
• Weekly hour guarantee: 32 hours per week.
• On call/weekend requirement: No. Weekend rotation and observed holiday availability may be required to meet patient needs.
• Mileage reimbursed between visits from first patient to last patient.
• Coverage areas include Malvern and Hot Springs, AR. Travel within the agency service area and to nearby agencies is required.

Impacting Patient Care

You will provide home health physical therapy services that relieve pain, restore or maintain function, and support patient independence. You must adhere to agency therapy standards of practice, complete timely documentation including progress notes and discharge summaries, participate in staff conferences, and access ongoing education to maintain clinical competency and state licensure.
